Job Summary

Felton Institute is seeking a highly skilled Clinical Case Manager to join our team in providing exceptional mental health services to older adults. As a key member of our multidisciplinary team, you will be responsible for delivering intensive case management services to individuals with severe psychiatric disorders.

Key Responsibilities
  • Participate in a multidisciplinary team to provide comprehensive care to clients
  • Conduct intakes and assessments for older adults with psychiatric and dual diagnosis disabilities
  • Develop and implement individualized treatment plans to address client needs
  • Provide individual and group therapy to decrease mental health symptoms and promote recovery
  • Coordinate with other service providers to ensure seamless care delivery
  • Maintain accurate and detailed records of client contacts and services provided
  • Participate in team meetings and case conferences to share knowledge and best practices
  • Answer a 24/7 after-hours crisis phone to support clients in crisis
Requirements
  • Master's degree or higher in Social Work or other Professional Counseling Programs
  • Active license with CA BBS, including ASWs, AMFTs, and APCCs
  • Minimum of 2 years of case management and mental health treatment experience for at-risk older adults
  • Strong recovery model approach to care and ability to form strong therapeutic relationships with clients
  • Excellent communication skills and ability to work effectively in a team environment
