About the Role

We are seeking a highly skilled Clinical Case Manager to join our team at Alpine Special Treatment Center. As a Clinical Case Manager, you will play a vital role in providing direct care to our clients, conducting thorough assessments, and developing treatment plans to ensure the best possible outcomes.

Key Responsibilities
  • Conduct comprehensive assessments to gauge individual risk and acuity levels
  • Develop and implement treatment plans in collaboration with clients and their support networks
  • Facilitate multidisciplinary treatment teams and work with admission and discharge staff to ensure effective treatment outcomes
  • Train staff on best practices and ensure compliance with regulatory requirements
  • Serve as a liaison between facility departments, clients, and stakeholders to ensure seamless treatment and positive experiences
Requirements
  • Master's degree in Psychology, Social Work, Marriage and Family Therapy, Counseling, or related field
  • Minimum of two years' experience working in a mental health program serving individuals with severe and persistent mental disabilities
  • CPR/BLS certification and National Provider Number (NPI) required
  • Preferred qualifications include a driver's license and registration as a Psychologist, MFT, LCSW, LPCC
