Nurse Specialist

At Yale New Haven Health, we are seeking a highly skilled and compassionate Registered Nurse to join our team. As a Registered Nurse, you will play a vital role in providing high-quality patient care to various patient populations within the framework of our YNHHS Home Hospital Model.
Key Responsibilities:- Provide protocol-driven assessment, care coordination, and interventions as appropriate informed by YNHHS Home Hospital protocols for diverse patient populations with a focus on oncology, medicine, and post-operative care.
- Exhibit sound clinical decision-making and critical thinking skills.
- Maintain and practice in accordance with nursing guidelines per state and hospital regulations.
- Facilitate admissions to YNHHS Home Hospital utilizing our guidelines.
- Serve as a role model for professional nursing practice.
- Utilize nursing knowledge to identify, prevent, and/or solve complex acute and/or recurring patient care problems.
- Share expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise.
- Support research efforts to advance knowledge and promote evidence-based practice.
- Perform other duties and responsibilities as assigned by the Director of Nursing.
- Ensure compliance with reporting requirements in RL Solutions for Sentinel Events and Serious Incidents.
- Participate in quality and safety investigations and root cause analyses.
- Take appropriate action to ensure patient safety as a result of any investigation.
- Serve as a resource in quality improvement processes and techniques.
- Graduation from an accredited School of Nursing.
- Current BCLS certification or must obtain within one month of hire.
- Registered Nurse is the foundational job description for all RNs.
- All RNs are encouraged to advance through the SPIRE clinical advancement program.
- Licensed and a currently registered nurse in the State of Connecticut.
- Clinical specialty certification highly encouraged.
- Requires prolonged standing; walking; use of sight, hearing, and touch.
- May be exposed to infectious diseases.
- Will be exposed to physical, mental, and emotional illness as well as end-of-life conditions and death.
- Moderate stress and risk of injury from patient care.
- Standing/walking for more than half of the day.
- Frequently bending, reaching, pushing, pulling, twisting, and lifting.
- Must be able to assist pushing/pulling/lifting patients ranging from five (5) to three hundred (300) pounds.
