Job Title: Clinical Evaluator

At Acentra Health, we are seeking a highly skilled Clinical Evaluator to join our team. As a Clinical Evaluator, you will play a critical role in ensuring the delivery of high-quality healthcare services to individuals with serious mental illness, intellectual disability, or related conditions.

Job Summary:

The Clinical Evaluator will be responsible for conducting face-to-face, clinical assessments of individuals in nursing homes or hospital settings. These assessments will determine if the individual meets the criteria for serious mental illness or intellectual disability/related conditions and will assess the level of care needs and provide recommendations for ongoing care.

Key Responsibilities:
  • Conduct accurate and comprehensive clinical assessments of individuals.
  • Recommend the appropriate level of care and service needs based on the individual's physical, behavioral, and psychological functioning.
  • Ensure compliance with program workflows and contractual requirements.
  • Develop and maintain strong relationships with individuals, families, and healthcare providers.
  • Stay up-to-date with the latest clinical practices and guidelines.
Requirements:
  • Active unrestricted licensure in North Carolina in a healthcare field such as a Licensed Psychologist, Licensed Clinical Social Worker, Licensed Clinical Mental Health Counselor, Licensed Psychological Associate, or RN (with master's degree in Psychiatric Nursing or relevant field).
  • Graduation from an accredited College with relevant Degree Program.
  • A clean MVR (Motor Vehicle Record) and reliable transportation to travel in the state of North Carolina.
  • 3+ years of practical experience in working with mental illness.
  • 3+ years of experience in working with individuals with intellectual disability, developmental disabilities, or related conditions.
  • Knowledge of the organization of medical records, medical terminology, and disease process required.
