Key Responsibilities
The Registered Nurse in Home and Community Based Services is responsible for delivering and overseeing nursing and personal care services to patients in their residences, as prescribed by their healthcare provider and in compliance with relevant laws and agency policies. The RN collaborates with other healthcare professionals to ensure continuity of care by coordinating the care plan and sharing information with physicians, agency staff, patients, families, case managers, and community resources.
Qualifications and Experience
- Deliver high-quality clinical services within the scope of practice and infection control standards, adhering to the care plan and collaborating with the patient's care team from admission to discharge.
- Conduct clinical nursing assessments in line with federal and state program requirements.
- Verify patient eligibility and medical necessity for services as defined by payer sources and agency policies.
- Develop and update individualized care plans in collaboration with other community providers.
- Ensure that the frequency and duration of care plans meet patient needs, making necessary adjustments with physician approval.
- A minimum of one year of experience as a Registered Nurse is preferred.
- Must possess a current RN license in the state of practice.
- Valid driver's license, vehicle insurance, and access to reliable transportation.
- Current CPR certification is required.
- KY: At least one year of clinical experience as a Registered Nurse is required. This requirement may be waived for RNs with recent clinical experience as an LPN.
- RN licensure must be unrestricted.
