We are seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to deliver comprehensive primary care services to patients of varying ages within a dynamic family practice environment.
Key Responsibilities
- Conduct thorough medical histories and physical examinations for patients with complex, acute, critical, and chronic health conditions.
- Utilize advanced diagnostic techniques to assess and understand medical conditions.
- Order and interpret a variety of diagnostic and therapeutic tests.
- Prescribe both pharmacological and non-pharmacological treatment options as appropriate.
- Monitor and evaluate the effectiveness of treatment interventions.
- Maintain detailed and accurate medical documentation.
- Collaborate with a multidisciplinary healthcare team to ensure comprehensive patient care and make referrals as necessary.
- Provide education to patients and their families regarding health conditions and treatment strategies.
- Facilitate decision-making processes for staff, patients, and families using evidence-based practices.
- Engage in the education and training of nurse practitioner or physician assistant students, as well as medical students.
- Perform additional duties as required by departmental needs.
Qualifications
For Nurse Practitioners:
Educational Requirements:
A master's degree in nursing and completion of an accredited Nurse Practitioner Program.
Experience Requirements:
A minimum of three (3) years of nursing experience.
Certifications:
Valid RN license to practice in Ohio with prescriptive authority required prior to employment.

For Physician Assistants:
Educational Requirements:
Graduation from an accredited Physician Assistant program.
Experience Requirements:
At least one year of experience as a Physician Assistant.
Certifications:
- Certification from the National Commission on Certification of Physician Assistants.
- Valid PA license to practice in Ohio.
Work Environment and Physical Demands
- Frequent sitting, standing, walking, and talking.
- Occasional bending, stooping, kneeling, and reaching overhead.
