Role Overview

About the Role
As an Advanced Practice Nurse at Abode Care Partners, you will be responsible for evaluating patient medical needs, treating chronic and urgent illnesses, and collaborating with healthcare professionals to maintain a comprehensive care plan. You will work closely with our interdisciplinary team to provide personalized care to patients in various settings, including skilled nursing facilities, assisted living, independent living, group homes, and private homes.

Responsibilities
  • Evaluate patient medical needs and develop individualized care plans
  • Provide evidence-based medical care, including diagnosing and treating acute and chronic illnesses
  • Collaborate with caregivers and other healthcare professionals to ensure seamless care coordination
  • Order and interpret laboratory and diagnostic testing as needed
  • Ensure patient medical records are accurate and up-to-date

Requirements
To be successful in this role, you will need:
  • Active state APRN license
  • Current National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of APRN experience preferred
  • Strong interpersonal communication skills – verbal and written
  • Excellent time management skills with accountability for practitioner's daily workflow
  • Proficiency in medical instruments and equipment
  • Demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
  • Licensed driver with valid automobile insurance
