Job Description
Mental Health Resource Center is a not-for-profit organization that provides mental health and behavioral health care services to the community. We are seeking a Care Coordinator Community Navigator to join our team.

Responsibilities
  • Care Coordination:
    • Identify and assist individuals admitted to local crisis stabilization units who are not effectively connected with the services and supports needed to access these services upon discharge.
    • Provide outreach and engagement to individuals referred from the crisis stabilization units.
    • Assess the individual's needs and coordinate a plan of care to meet these needs.
    • Advocate for acquisition of the services and resources necessary for successful transition of care.
  • Documentation and Reporting:
    • Ensure client records are current, accurate, and include documentation of all activities, contacts, and communications.
    • Obtain consents and releases of information for past medical, psychiatric, or substance abuse treatment, when indicated.
  • Assess and monitor for risk, symptoms of trauma, and indications of abuse, neglect and/or abandonment.

Qualifications
  • Bachelor's Degree in Social Work or a related Human Services field from an accredited university or college.
  • One year experience working in human services or mental health related field.
  • Proficiency in Microsoft Office, Outlook, and use of the Internet.
  • Valid Florida driver's license and insurance coverage equal to or exceeding 50,000/100,000/50,000 split limits.

Position Details
This is a Full Time Days position: Monday through Friday, 8:00am to 4:30pm.
