Home Health RN Full Time Position

Job Summary:
Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.
Responsibilities:
- Develop and implement individualized care plans in accordance with physician orders and client needs.
- Provide education and support to clients and their families to promote optimal health outcomes.
- Collaborate with primary caregivers to ensure seamless care transitions and optimal functioning levels.
- Coordinate with the client's physician and other caregivers to ensure timely and effective care.
- Provide skilled nursing care in patients' homes, utilizing a range of skills including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.
Requirements:
- Active RN license in the state of application.
- Valid CPR and driver's license.
- Preferred: Medicare skilled nursing experience, basic understanding of Oasis, and 1-year RN experience in a healthcare setting.
