Job Summary:
Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse RN Full Time. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.
Key Responsibilities:
- Develop and implement individualized care plans in accordance with physician orders and client needs.
- Provide education and training to clients and their family members on disease management, medication administration, and other relevant topics.
- Conduct thorough assessments and evaluations to identify client needs and develop effective care plans.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Document all patient interactions, assessments, and care plans in a timely and accurate manner.
Requirements:
- Active RN license in the state of application.
- Valid CPR certification and driver's license.
- 1 year of experience in a healthcare setting, preferably in home health care.
- Basic understanding of OASIS and Medicare Skilled Nursing guidelines.
