Position Overview:

As a Home Health Aide at A Place at Home, you play an essential role in enhancing the well-being of our clients. Your responsibilities will include delivering empathetic, high-quality assistance with daily living activities (ADLs) tailored to each client's unique care plan. Under the guidance of our registered nursing staff, you will ensure that our clients receive exceptional care in the comfort of their homes.

Key Responsibilities:

  • Providing support with ADLs, including bathing, dressing, grooming, and toileting.
  • Assisting with meal preparation and planning, while adhering to dietary restrictions as needed.
  • Offering medication reminders in alignment with established care plans.
  • Conducting light housekeeping duties such as dusting, vacuuming, and laundry to maintain a tidy and welcoming environment for our clients.
  • Assisting with mobility and transfers, ensuring the safety of clients at all times.
  • Providing companionship and nurturing relationships with clients.
  • Documenting and reporting any changes in clients' physical condition, behavior, or living situations to the supervising nurse.
  • Adhering to HIPAA regulations and safeguarding the privacy of all client personal health information (PHI).
  • Engaging in ongoing training and professional development opportunities.

Skills and Qualifications:

  • Completion of a state-approved Home Health Aide certification program.
  • Excellent communication skills, both verbal and written.
  • Empathetic and compassionate nature with a strong commitment to enhancing the lives of others.
  • Ability to work independently while demonstrating patience and understanding.
  • Physically capable of performing job duties, including lifting and physical movement as required.
  • Possession of reliable transportation and a valid driver's license.

Education and Experience Requirements:

  • High school diploma or equivalent.
  • Valid Home Health Aide certification is mandatory.
  • Successful completion of a background check and drug screening.

Physical Demands:

This position requires moderate physical activity, including bending, lifting, and walking.
