We are looking for a motivated and proactive Client Support Coordinator (Case Manager/Social Worker).

The Wood County Board of Developmental Disabilities (WCBDD) oversees services for over 1,000 individuals with developmental disabilities in Wood County. Our mission is to support, empower, and inspire. The ideal candidate will be a highly self-driven problem solver, who excels in teamwork, time management, and leadership capabilities.

The Client Support Coordinator will handle a caseload of individuals, be responsible for crafting the individual service plan, and monitor the services to ensure health and safety.

Client Support Coordinator

  • Normal Working Hours: M-F, 8:00am - 4:30pm
Key Responsibilities:
  • Organize and lead Individual Service Plan (ISP)/ Individual Family Service Plan (IFSP) meetings using a person-centered approach.
  • Develop and modify the ISP/IFSP as necessary, including service authorizations for individuals based on assessed needs and utilizing the Interdisciplinary Team process.
  • Responsible for executing, documenting, and monitoring ISP data, including narratives, assessments, and other related paperwork/materials necessary to comply with state/federal regulations and Board policies/procedures.
  • Evaluate, coordinate, and ensure the delivery of services aligned with individual needs as consented to in accordance with the Individual Service Plan.
  • Direct all services and behavioral programs.
  • Guarantee complete and accurate documentation of all programs and review service delivery.
  • Ensure individual/guardian choice in provider selection, assisting as needed in accordance with HCBS guidelines and/or Board Policies or Procedures.
  • Collaborate with local educational institutions and Early Intervention providers.
  • Act as a liaison among various Board components, families, external agencies, the community, and contracted service providers.
  • Provide information and assistance in securing services for eligible children and adults in educational, vocational, housing, and other community-based services.
  • Advocate for eligible individuals through direct contact. Document all service and support activities through Targeted Case Management, based on identified needs and services.
  • Offer information and referral services to all interested individuals. Coordinate services for eligible individuals through direct contact and share information intra-agency as well as with other agency providers.
  • Review Unusual Incidents and track and monitor for trends and patterns.
  • Assist individuals/families/providers in completing all required documentation for initial Waiver enrollment packet, ensuring all required forms are consistent and complete before submission to the Ohio Department of Developmental Disabilities.
  • Help individuals/families/providers in coordinating services based on multifactor assessments.
  • Coordinate completion of initial and annual comprehensive evaluations/assessments in accordance with Waiver criteria.
  • Maintain ongoing communication with individuals/families/providers regarding the status of enrollment packets, services being delivered, ISP revisions, and redeterminations.
  • Maintain periodic contact and report, as needed, of ongoing communication with individuals/families/providers to ensure that all services identified in the ISP are being received/provided.
  • Stay informed about HCBS Waivers.
  • Provide technical assistance to individuals/families/providers regarding billing, documentation, amended plans, and redeterminations.
  • Assist in developing success plan strategies and Positive Supports First, monitor the effectiveness of such programs, and attend Transdisciplinary Meetings.
  • Assist in completing forms for restrictive measures to be reviewed by the Human Rights Committee.
  • Share 24-hour on-call responsibilities with other Client Support Coordinators in crisis intervention and stabilization.
  • Client Support Coordinators must be knowledgeable about Major Unusual Incidents (MUI) and reporting criteria, along with completing necessary MUI paperwork.

Education/Experience Requirements:
  • Bachelor's degree required.
  • Preferred degree in Social Work, Psychology, Special Education, or related fields.
  • Minimum of 2 years' experience working directly with individuals with developmental disabilities preferred.

Minimum Qualification Standards:
  • Ohio Department of Developmental Disabilities Certification, if applicable.
  • Must obtain and maintain any training, licenses, registration, or certification mandated by the Board.
  • Maintain a valid Driver's License for the purpose of transporting individuals served and/or operation of a Board-owned vehicle.
  • Must be able to travel locally and statewide.
  • Must obtain and maintain a library card from an Ohio public library.
  • All new employees must have a medical examination and a one-step PPD test to determine fitness for duty.
  • Pre-employment and random drug and alcohol testing required.
  • Receipt of satisfactory criminal background reports as required.
  • Must be able to lift, carry, and move individuals receiving services safely.
  • Must be able to interact appropriately with individuals who may display aggressive behaviors.

Physical Demands and Working Conditions:
  • Work is primarily performed in an indoor activity-based setting, a vehicle, and/or occasionally in a community setting.
  • Primary functions require sufficient physical ability and mobility to stand or sit for prolonged periods; to occasionally stoop, bend, kneel, crouch, reach, and twist; to lift, carry, push, and/or pull moderate amounts of weight; to operate office equipment requiring repetitive hand movement and fine coordination.
  • Required to safely move/transfer individuals of varying weights using proper lifting techniques and equipment.
