Mental Health Case Manager

Harbor is seeking a skilled CPST/TBS Provider to join our mental health team in Bowling Green. This full-time position (40 hours per week, Monday-Friday) offers a rewarding opportunity to make a positive impact in the lives of our clients.

Key Responsibilities:
  • Accurately and timely document client interactions, including activity/progress notes, ISPs, DPS logs, and incident report forms.
  • Assess client needs and develop a strength-based ISP in collaboration with the client, family, and/or significant others.
  • Provide crisis prevention and intervention services.
  • Offer support, education, and consultation to clients, families, and/or significant others.
  • Monitor clients to identify and minimize the effects of psychiatric symptoms.
  • Assist clients and families in increasing social support skills and networks.
  • Coordinate indicated evaluations and assessments by internal and/or external providers.
  • Coordinate and monitor all services identified in the ISP.
  • Facilitate access to essential community resources.
Requirements:
  • A minimum of a High School Diploma and 3 years of experience are required. A Bachelor's degree in Counseling, Social Work, Psychology, Nursing, or a closely related behavioral/mental health field is preferred.
  • Proficiency in computer use, including Microsoft Word, is essential.
  • Successful completion of required training in basic competencies, including mental illness or substance use disorder treatment and recovery, psychiatric and substance use disorder symptoms, crisis response procedures, and de-escalation techniques, is mandatory.
  • Non-licensed staff must complete 20 hours of CEUs every 24 months.
  • Reliable transportation, a valid driver's license, an acceptable driving record, and insurability through Harbor's vehicle insurance carrier are required.
  • CPR/First Aid and NCI/CPI Certifications are required within 90 days of employment.
