Key Responsibilities:- Assess and evaluate patient needs, developing personalized care plans that address physical, psychosocial, and environmental factors.
- Coordinate and implement care plans, working collaboratively with patients, families, and healthcare teams to ensure seamless care delivery.
- Communicate effectively with patients, families, and healthcare professionals to ensure timely and accurate information exchange.
- Re-evaluate patient needs regularly, revising care plans as necessary to ensure optimal outcomes.
- Current, unrestricted state RN license
- 1+ year experience in a clinical care setting as a nurse
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Associates Degree or Bachelor's Degree in Nursing is preferred
- Previous Home Health or Hospice Experience & Oasis experience is required
