Key Responsibilities:
  • Assess and evaluate patient needs, developing personalized care plans that address physical, psychosocial, and environmental factors.
  • Coordinate and implement care plans, working collaboratively with patients, families, and healthcare teams to ensure seamless care delivery.
  • Communicate effectively with patients, families, and healthcare professionals to ensure timely and accurate information exchange.
  • Re-evaluate patient needs regularly, revising care plans as necessary to ensure optimal outcomes.
Requirements:
  • Current, unrestricted state RN license
  • 1+ year experience in a clinical care setting as a nurse
  • Able and willing to travel within branch/office coverage area
  • Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
  • Associates Degree or Bachelor's Degree in Nursing is preferred
  • Previous Home Health or Hospice Experience & Oasis experience is required
