Job Summary

We are seeking a highly skilled and compassionate Recovery Navigator to join our team at Wisconsin Community Services. As a Recovery Navigator, you will play a critical role in supporting individuals with significant substance use and co-occurring mental health needs. Your primary responsibility will be to work collaboratively with our team and the individual to develop a comprehensive care plan, facilitate regular recovery monitoring and meetings, and ensure they receive quality care from our team and community service providers.

Key Responsibilities
  • Engage individuals in care and services using a strength-based, trauma-informed, person-centered, and culturally intelligent approach.
  • Closely collaborate with our Lead Recovery Navigator and WCS Hub Team to serve individuals referred to our program.
  • Provide direct service to approximately 12 individuals weekly, including completing a Recovery Life Story Tool and Assessment (RLST/PRP) and updating it every 90 days.
  • Facilitate monthly BAM monitoring/team meetings and coordinate care with providers of services.
  • Verify services are provided and contact each member of the individual's care team weekly to review progress and areas of ongoing need.
  • Work closely with the individual to develop a team of informal and formal supports based on their preference.
  • Assist with planning and implementing monthly support groups or other educational events for individuals in our program.
  • Provide transportation and attend appointments with the individual, advocating with them to meet their identified needs.
  • Support crisis response and develop strategies to meet immediate needs, including developing a comprehensive relapse prevention plan with the individual.
  • Complete satisfaction surveys with individuals in our program and follow all WCS Hub policies and procedures.
Requirements
  • High school or GED/HSED required; Bachelor's degree in Social Work, Psychology, Nursing, or Social Services preferred.
  • Knowledge and experience serving people with substance use needs.
  • Working knowledge of computer applications, including Windows and Microsoft Word.
  • Ability to display cultural competence and respond respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
  • Valid driver's license, automobile, and insurance sufficient to meet agency requirements.
Working Conditions

This role requires working in an office and community environment, including going into homes, court, and other locations to serve adults enrolled in our program. Travel is required, and you must be comfortable interacting with adults who have significant substance use needs. A flexible work schedule is required to meet the needs of staff and the individuals we serve.
