Job Title: Home Health Occupational Therapist

LHC Group is seeking a skilled Home Health Occupational Therapist to join our team in Kaufman, Texas.

About the Role:

We are looking for a dedicated and compassionate Occupational Therapist to provide high-quality care to our patients in their homes. As a Home Health Occupational Therapist, you will work closely with our interdisciplinary team to develop and implement personalized care plans that promote independence, mobility, and overall well-being.

Responsibilities:
  • Conduct comprehensive assessments of patients' physical and functional abilities to identify areas for improvement.
  • Develop and implement individualized care plans that address patients' needs and goals.
  • Provide occupational therapy interventions, including exercises, activities, and education, to promote patients' independence and mobility.
  • Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care.
  • Document patient progress and outcomes in accordance with agency policies and regulatory requirements.
Requirements:
  • Current licensure as an Occupational Therapist in the state of Texas.
  • Current CPR certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to travel to patients' homes in the Kaufman area.
