We are seeking a skilled LVN Clinical Liaison to join our team at AccentCare, Inc. as a Clinical Liaison Sales LVN. This role will involve working with patients in hospitals and nursing facilities to discuss services and transition to home care.
Key Responsibilities:
- Meet with patients to discuss services and transition to home care
- Act as a liaison for clinical appropriateness to internal Branch Directors
- Coordinate home health services with physicians as needed
- Partner with Account Executives to provide clinical education to referral sources
- Review and assist with hospital hold list
Requirements:
- Associates or Bachelor's degree in nursing with LVN/LPN license
