This role is essential in delivering licensed nursing services to our residents, adhering to the standards set forth by Azpira Place of Lake Zurich.

Key Responsibilities:

  • Administer medications to residents in accordance with Federal and State regulations.
  • Conduct admissions for new residents, including all necessary assessments and documentation.
  • Develop and implement acute and long-term care plans.
  • Oversee clinical system tracking, such as wound care and infection control, under the guidance of the Health and Wellness Director.
  • Provide health-related consultations within the community.
  • Communicate any changes in residents' health to physicians and family members, ensuring proper documentation.
  • Respond promptly to emergencies and maintain effective communication with all departments.
  • Facilitate educational meetings for residents and staff.
  • Uphold the confidentiality of resident information.
  • Participate in all required training and staff meetings.

Safety Commitment: Strive to maintain a safe working environment by preventing accidents and preserving equipment.

Qualifications:

  • Current Licensed Practical or Vocational Nurse's License in the practicing state.
  • CPR certification is mandatory.
  • Experience in senior care or a related field is preferred.
  • Ability to work collaboratively within a team and manage multiple tasks effectively.
  • Strong verbal and written communication skills.
  • Demonstrated dedication, compassion, and energy.
