Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team at Alden Long Grove Rehabilitation and Health Care Center. As a LPN, you will be responsible for providing direct nursing care to our customers and supervising the day-to-day nursing activities performed by nursing assistants.

Key Responsibilities
  • Provide direct nursing care to customers, including administering medications, performing routine charting duties, and completing required assessments.
  • Supervise nursing assistants and ensure they are following established policies and procedures.
  • Develop and implement individualized care plans for each customer, taking into account their unique needs and goals.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and address any concerns or issues that may arise.
  • Participate in ongoing education and training to stay up-to-date on the latest nursing practices and technologies.
Requirements
  • Current, unencumbered license to practice as a LPN in the State of Illinois and/or Wisconsin.
  • Current CPR certification or willingness to become certified within two weeks of employment.
  • Knowledge of nursing and medical practices and procedures.
  • Ability to read, write, and speak the English language in an understandable manner.
  • Leadership and supervisory ability, with a willingness to work harmoniously with other personnel.
Working Conditions
  • Must be able to move intermittently throughout the workday.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately.
  • Must function independently, have flexibility, personal integrity, and the ability to work effectively with customers, family members, personnel, and support agencies.
