About the Role

We are seeking a skilled Registered Nurse to join our Hospice House/Inpatient Care team. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families during their end-of-life journey.

Key Responsibilities
  • Plan and deliver patient care utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation.
  • Effectively interact with patients, families, and other healthcare professionals to ensure seamless care coordination.
  • Collaborate with the interdisciplinary team to develop and implement individualized patient care plans.
  • Provide education and support to patients and their families to promote optimal health outcomes.
  • Participate in quality improvement initiatives to enhance patient care and services.
Requirements
  • Graduate of an accredited nursing school with an Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN) degree.
  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Two years of acute care nursing experience preferred.
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
  • CPR certification.
  • Computer skills sufficient to properly document patient services and care.
  • Ability to drive during daytime, nighttime, or inclement weather.
  • Valid Driver's License with a safe driving record.
  • State Minimum Automobile Insurance Coverage.
