Key Responsibilities:
  • Deliver speech therapy services in accordance with state regulations and the established care plan, collaborating with the healthcare team.
  • Assess patients' functional abilities through diagnostic evaluations and assist in creating tailored therapy plans.
  • Facilitate effective communication for patients, adapting techniques to their individual needs.
  • Monitor and document patient progress, reporting changes to the supervising nurse or physician.
  • Educate patients, families, and caregivers on speech therapy techniques and their roles in the process.
  • Conduct patient visits as per the treatment plan, documenting progress in compliance with company policies.
  • Complete evaluations and develop individualized therapy plans, reporting outcomes to the supervising nurse promptly.
  • Engage in team meetings and contribute to committees as needed.
  • Provide training sessions for agency staff upon request.
  • Submit written summaries of patient conditions to the supervising nurse and/or physician regularly.
Qualifications:

Licensure Requirements:
  • Current state licensure in Speech Therapy
  • Valid CPR certification
  • Possession of a valid driver's license and vehicle insurance, with access to reliable transportation
