We are excited to announce that we are hiring a Nurse Case Manager to join our team at Advantage Care Services If you are passionate about making a difference in the lives of others, we want to hear from you

Key Responsibilities:

  • Assess client needs and develop individualized care plans to address those needs.
  • Work collaboratively with our multidisciplinary team to ensure seamless care coordination.
  • Monitor and report on client progress, identifying areas for improvement and implementing changes as needed.

Requirements:

  • Active RN licensure in the state of Texas.
  • Minimum 1 year of experience in case management or a related field.
  • Exceptional communication and organizational skills.

Salary: $60,000 - $75,000 per year, depending on experience.
