Job Summary

We are seeking a skilled Care Coordinator Specialist to join our team at Bayview Hunters Point Foundation for Community Improvement. This is a unique opportunity to make a meaningful impact in the lives of individuals and families struggling with mental health issues.

About the Role

In this role, you will be responsible for providing comprehensive case management, mental health, and substance use services to clients. You will work closely with a multidisciplinary team to develop and implement individualized care plans that address the complex needs of our clients.

Key Responsibilities

  • Assess clients' needs and functioning, and present cases in treatment team meetings as needed.
  • Act as a client advocate, interfacing with housing and support agencies to ensure clients receive the necessary resources and services.
  • Develop and maintain rapport and cooperative relationships with community agencies to benefit clients.
  • Provide individual and group supportive counseling and psycho-education to assist clients in coping with mental illness and/or substance use.
  • Facilitate conflict resolution among program clients as needed, and provide crisis intervention when necessary.
  • Collaborate with clinical and medical staff to coordinate client care.

Requirements

To be successful in this role, you will need:

  • A minimum of two years of related experience or a bachelor's degree.
  • Previous experience working in a social service/mental health environment, preferably with mental health and/or substance use clients.
  • A current California driver's license, car insurance, and a driving record acceptable to our insurance carrier.
