About the Position
We are seeking a skilled Care Coordinator I to join our team at Bayview Hunters Point Foundation for Community Improvement. In this role, you will work closely with clients to provide comprehensive case management, mental health, and substance use services. Your primary duties will include assisting clients to access healthcare services, establishing and maintaining housing, and increasing social contacts and engaging in constructive activities.

Key Responsibilities:
  • Develops and maintains rapport and cooperative relationships with community agencies for the benefit of the clients.
  • Provides individual/group supportive counseling and psycho-education to assist clients in coping with mental illness and/or substance use.
  • Monitors and maintains documentation of client services; ensures compliance with Agency, county, state, and federal policies and procedures, standards, and guidelines.

Requirements:
  • A minimum of two (2) years of related experience or a bachelor's degree is required.
  • Previous experience working in a social service/mental health environment, preferably with mental health and/or substance use clients.
  • Knowledgeable about and actively supports integrated mental health, physical health, and substance use treatment for improving care for the whole person.

Benefits:This position offers a competitive salary, opportunities for professional growth and development, and a supportive work environment. The estimated salary for this position is between $21.38 - $24.59 per hour, with a $2,000 sign-on bonus for external candidates.
