Job Title: Travel Nurse RN Case Manager

Stability Healthcare is seeking a skilled Travel Nurse RN Case Manager to join our team at SSM Health Saint Mary's Hospital-Madison in Madison, Wisconsin.

Job Summary:

We are looking for a dedicated and compassionate Travel Nurse RN Case Manager to provide high-quality patient care and support to our patients and families. As a Travel Nurse RN Case Manager, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities:
  • Assess patient needs and develop individualized care plans
  • Collaborate with healthcare team members to ensure seamless care delivery
  • Provide education and support to patients and families
  • Document patient progress and outcomes
  • Participate in quality improvement initiatives
Requirements:
  • Current RN licensure in Wisconsin
  • Minimum 1 year of experience in case management or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Basic computer skills and proficiency in electronic medical records
