Job Title: RN Case Manager

SSM Health is seeking experienced RNs for inpatient Case Management opportunities at SSM Health St. Mary's Hospital – Madison, WI.

Job Highlights:
  • Full and part-time positions available
  • May be eligible for a $10,000 Sign on Bonus for full-time and $5,000 Sign On Bonus for part-time
About the Role:

The RN-Case Manager supports our organization by ensuring patients are discharged from the hospital safely, identifying any barriers and working through those with the patient and care team.

Responsibilities and Requirements:
  • Assesses, develops, plans, implements, and evaluates patients' discharge planning needs to next level of care in collaboration with interdisciplinary team
  • Ensures compliance with Condition of Participation (CoP) discharge planning
  • Identifies and escalates gaps/barriers related to inpatient and observation length of stay
  • Applies the existing body of evidence-based practice and scientific knowledge in healthcare to nursing practice
Education and Experience:
  • Graduate of accredited school of nursing or education equivalency for licensing
  • Three years' acute care experience
Physical Requirements:
  • Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs
  • Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements
Required Professional License and/or Certifications:
  • State of Work Location: Illinois – Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
  • State of Work Location: Missouri – Registered Nurse (RN) Issued by Compact State
  • State of Work Location: Oklahoma – Registered Nurse (RN) Issued by Compact State
  • State of Work Location: Wisconsin – Registered Nurse (RN) Issued by Compact State
