At Yale New Haven Health, we are seeking a highly skilled Paramedic Transfer Facilitator to join our team. As a key member of our Patient Access Transfer Center, you will play a critical role in facilitating the transfer of patients between our hospitals and other healthcare facilities.
Key Responsibilities:- Patient Throughput: Obtain accurate and pertinent medical history, direct referring physicians to appropriate service lines, and work collaboratively with Bed Associates and Clinical Bed Managers to ensure proper bed placement.
- Customer Service: Develop effective relationships with support staff and physicians to ensure patient flow and bed placement, communicate effectively, and create an employee and physician experience that exemplifies caring, understanding, efficiency, and overall quality of service.
- Census Verification: Properly document and follow policy and procedures related to processes developed, work with Bed Associates and Clinical Bed Managers to ensure proper placement at all hospitals.
- Resource Management: Maintain strong knowledge of EPIC and CRM applications, multitask, and make decisions related to physician service lines and patient placement.
- EMT-P Certification required
- Minimum of two years of experience as a paramedic, with experience in healthcare, clinical practice, triaging, and transfer experience
- Demonstrated leadership experience with excellent judgment and prioritization skills
