Healthcare Transfer Facilitator

The Transfer Facilitator plays a crucial role in facilitating transfer requests from referring physicians, including direct admissions, telehealth, and Consult Connect.
Working closely with Clinical Bed Managers, Off Shift Administrators, Attending Physician Staff, Inpatient Bed Resources, and Referring Physicians, the Transfer Facilitator expedites transfer admissions and direct admissions to all five hospitals - Yale New Haven with the St Raphael's Campus, Bridgeport, Greenwich, Lawrence + Memorial, and Westerly.
The Transfer Facilitator is responsible for understanding what diagnosis is appropriate for each individual hospital and directing placement accordingly.
Accountability for improving the quality and efficiency of the transfer process and the satisfaction of internal and external customers is a key responsibility.
The Transfer Facilitator must understand and apply knowledge of policies and procedures for each hospital when completing processes.

Obtaining accurate and pertinent medical history is essential for Patient Throughput. The Transfer Facilitator documents and reports on timelines of patient activity for efficient throughput.
Understanding bed availability at each individual hospital and the complex admissions with different processes and algorithms at each hospital is critical.
Customer Service is a top priority, and the Transfer Facilitator develops effective relationships with all support staff and physicians to ensure patient flow and bed placement.
Effective communication and creating an employee and physician experience that exemplifies caring, understanding, efficiency, and overall quality of service are essential.
Census Verification requires properly documenting and following policy and procedures related to processes developed.
Resource Management involves maintaining a strong knowledge of EPIC and CRM applications, as well as professional communication skills and the ability to document appropriate information accurately.
Supporting team building and communication of the department is also a key responsibility.
The Transfer Facilitator must participate in designated PFAS workgroups to support team building and communication.
Continuing Education requires yearly completion of 20 hours of continuing medical education (CME) through approved credentialed providers.
A high school graduate or equivalent is required, as well as experience in healthcare, including clinical practice, triaging, and transfer experience.
Proficiency in typing and data entry is also necessary.
