The Registered Nurse (RN) will provide care and services for a group of patients, visiting them in their homes, skilled nursing facilities, and assisted living facilities. This role is a contributing member of the interdisciplinary team, providing expertise on psych-social assessments, interventions, and community resources.
Responsibilities- Provide care and services for patients in their homes and other settings
- Collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership to deliver high-quality care
- Work independently and on your own from day-to-day, managing visits, documentation, meetings, and communication
- Manage time effectively to prioritize and triage multiple concerns
- Learn and utilize an electronic medical records system
- Travel across the service area with reliable transportation, valid driver's license, and auto insurance
- Current Registered Nurse (RN) licensure in good standing in the state(s) of practice
- Ability to work well within a team environment
- Strong communication and time management skills
- Ability to learn and adapt to new systems and processes
