Position Overview

The primary objective of this role is to deliver direct nursing care to residents while overseeing the daily nursing operations carried out by nursing assistants.

This oversight must align with the prevailing federal, state, and local standards, guidelines, and regulations governing our facility, as directed by the Director of Nursing Services or Nurse Supervisor, to ensure the highest quality of care is consistently maintained.

Key Responsibilities

Administrative Duties
  • Oversee the daily activities of nursing assistants in compliance with current regulations and guidelines.
  • Ensure adherence to established policies and procedures by all nursing personnel under your supervision.
  • Review and recommend updates to departmental policies and procedure manuals.
  • Collaborate with nursing staff and support personnel in planning services, programs, and activities for shifts.
  • Maintain the Nursing Service Procedures Manual to reflect current practices.
  • Ensure compliance with procedures outlined in the Nursing Service Procedures Manual by all nursing staff.
  • Prepare written and oral reports regarding shift activities as required.
  • Coordinate with other resident services to maintain a comprehensive care regimen.
  • Ensure all nursing personnel adhere to their job descriptions.
  • Engage in the development and implementation of the facility's quality assurance program for nursing services.
  • Participate in inspections conducted by authorized government agencies as requested.
  • Review and update resident discharge plans as necessary.
  • Assist in planning nursing services for resident discharge as needed.
  • Interpret departmental policies and procedures to staff, residents, visitors, and government agencies as required.
  • Manage the admission, transfer, and discharge processes for residents.
  • Complete necessary accident and incident reports.
  • Perform administrative tasks such as completing medical forms, reports, evaluations, and charting.
  • Maintain confidentiality regarding user ID codes and resident information.
  • Report any suspected violations of confidentiality or unauthorized access attempts.
Charting and Documentation
  • Complete and file necessary recordkeeping forms upon resident admission, transfer, and discharge.
  • Encourage physicians to review and sign treatment plans and progress notes as per established policies.
  • Document telephone orders from physicians accurately.
  • Transcribe physician orders to resident charts and care plans as required.
  • Maintain detailed and informative nurses' notes reflecting care provided and resident responses.
  • Complete accident and incident reports and submit them as required.
  • Document any accidents or incidents involving residents following established procedures.
  • Record changes in diet orders and communicate with the Food Services Department.
  • Report discrepancies in physician orders or charting errors to the Nurse Supervisor.
  • Complete transfer forms according to established procedures.
  • Perform routine charting duties as required.
  • Sign and date all entries in the resident's medical record.
Medication Administration
  • Prepare and administer medications as prescribed by physicians.
  • Verify resident identity prior to medication administration.
  • Ensure medications are not administered to the wrong resident.
  • Maintain adequate supplies of medications and report needs to the Nurse Supervisor.
  • Order medications and supplies as necessary following established policies.
  • Ensure accuracy in narcotic records for your shift.
  • Notify the Nurse Supervisor of any discrepancies in drug and narcotic records.
  • Review medication cards for completeness and accuracy.
  • Dispose of medications and narcotics according to established procedures.
Personnel Management
  • Report absenteeism to the Nurse Supervisor.
  • Assist in developing work assignments for nursing personnel.
  • Provide leadership and support to nursing staff on your unit/shift.
  • Foster a collaborative working relationship with inter-departmental personnel.
  • Create a warm and positive environment for residents and staff.
  • Administer disciplinary actions fairly and without bias.
  • Exchange nursing reports at the beginning and end of shifts.
  • Report occupational exposures to hazardous materials according to facility policies.
  • Ensure proper security measures for departmental computer workstations.
Nursing Care Responsibilities
  • Inform nursing staff of new admissions and their details.
  • Prepare rooms for new residents prior to their arrival.
  • Welcome newly admitted residents and assist them as needed.
  • Participate in orienting new residents and their families to the facility.
  • Accompany physicians during rounds as necessary.
  • Arrange for diagnostic and therapeutic services as ordered by physicians.
  • Consult with physicians regarding resident care and treatment.
  • Review resident charts for treatment and medication orders.
  • Implement established nursing objectives and standards.
  • Conduct periodic checks to ensure proper treatment administration by nursing assistants.
  • Notify physicians of any accidents or changes in resident conditions.
  • Administer specialized services as required.
  • Engage with residents and family members as necessary and report issues to the Nurse Supervisor.
Professional Development
  • Participate in external training programs.
  • Attend annual in-service training programs as scheduled.
  • Engage in continuing education to stay updated in the nursing field.
Safety and Sanitation
  • Monitor staff adherence to safety regulations in equipment use.
  • Ensure availability of personal protective equipment for staff.
  • Maintain cleanliness in work areas and resident care rooms.
  • Enforce proper hand hygiene among nursing personnel.
  • Ensure participation in fire safety and disaster preparedness drills.
  • Follow established infection control procedures when necessary.
  • Ensure proper use and disposal of personal protective equipment.
  • Report any safety concerns regarding labels and MSDSs.
Equipment and Supply Management
  • Maintain adequate stock levels of medications and supplies on your unit/shift.
  • Ensure safe operation of nursing service equipment by all personnel.
  • Keep MSDSs updated for hazardous chemicals used in nursing services.
Care Plan and Assessment
  • Review care plans daily to ensure appropriate care delivery.
  • Inform the Nurse Supervisor of necessary care plan updates.
  • Ensure nurses' notes reflect adherence to care plans.
  • Review resident care plans for goals and necessary revisions.
  • Ensure nursing assistants are familiar with resident care plans.
Miscellaneous Responsibilities
  • Provide data to the Quality Assurance & Assessment Committee as requested.
Supervisory Responsibilities

As an LPN/LVN, you are entrusted with the administrative authority, responsibility, and accountability necessary for fulfilling your assigned duties.

Qualifications

Education and Experience: Must possess a Nursing Degree from an accredited institution or be a graduate of an approved LPN/LVN program. Must hold a current, unencumbered license to practice as an LPN/LVN in this state.

Language Skills: Proficiency in reading, writing, speaking, and understanding English is required. Ability to comprehend technical procedures and policy manuals is essential.

Mathematical Skills: Ability to apply concepts such as fractions, percentages, and ratios to practical situations.

Reasoning Ability: Must demonstrate knowledge and skills necessary to provide age-appropriate care. Ability to make independent decisions when necessary.

Certificates, Licenses, Registrations: Must maintain a current, unencumbered license to practice as an LPN/LVN in this state and hold current CPR certification.

Physical Demands: Must be able to move throughout the workday and manage the physical demands of the position.
