Job Summary

We are seeking a highly skilled Patient Care Coordinator - Registered Nurse to join our team at Siouxland Community Health Center. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and support services.

Key Responsibilities
  • Triage and Communication: Respond to patient phone calls, triage their needs, and document interactions accordingly.
  • Scheduling and Referrals: Schedule appointments, make referrals, and communicate with providers as needed.
  • Electronic Medical Records: Access, update, and maintain accurate patient information in our electronic medical record system.
  • Medication Refills: Refill patient medications according to SCHC protocol and policy.
  • Documentation and Education: Prepare, update, and document patient information, as well as educate patients on health-related topics and distribute relevant health information.
  • Follow-up and Coordination: Ensure proper follow-up is completed and documented for patients seen in emergency departments or hospitals, and coordinate with hospitals to confirm receipt of patient records.
  • Process Improvement: Utilize critical thinking skills to evaluate patient needs and suggest improvements in departmental workflow and processes.
  • Quality Initiatives: Participate in process improvement events, report outs, and support the PI and Quality teams in their initiatives.
Requirements
  • Current RN licensure
  • Current BCLS and Mandatory Reporter certification
  • Solid computer skills
  • Multi-Physician Family Practice experience preferred
  • Electronic Medical/Health Records (EMR) experience a definite plus
