Job Summary

We are seeking a highly skilled Pharmacist to join our team at Penn Medicine. As a Clinical Pharmacy Leader, you will be responsible for managing the clinical and production pharmacy services provided to assigned patients. Your primary focus will be on ensuring the safety and quality of patient care, while also providing in-service programs to personnel on any aspect of drug therapy and related infusion devices.



Key Responsibilities

  • Provides clinical decisions regarding patient-specific pharmaceutical care, including assessment of patient needs, review of medication profiles, and evaluation of duplicate therapies and drug interactions.
  • Ensures the safety of patients and pharmacy personnel, and develops patient databases and documentation in medical records.
  • Participates in Penn Home IT Continuous Quality Improvement program and accepts physician prescription orders, evaluating their appropriateness for dosage, drug choice, and potential interactions.
  • Monitors patient response to treatment, laboratory tests, and provides recommendations to improve or adjust therapy based on lab evaluations.


Requirements

  • Pharmacist (Required) Pennsylvania State Pharmacist License required.
  • Bachelor of Arts or Science (Required) or equivalent experience.
  • Graduate of an accredited School of Pharmacy or equivalent experience.
  • 2+ years experience with infusion therapies in hospital and/or home settings, and experience in patient care and medication counseling, preparation of parenteral products, drug therapy monitoring, clinical laboratory evaluation, drug and medical supply procurement and quality assurance.
