• Establish and maintain strong collaborative relationships with existing Consumer and Family organizations to encourage active participation in Optum/Health Plan activities.

• Communicate vital information regarding plans to consumers and consumer-operated organizations, as well as families and family organizations.

• Identify and connect with community leaders from ethnic minority groups to create culturally competent programs that emphasize recovery and resilience.

• Participate in internal subcommittees as directed and engage in Optum and Health Plan Quality Review processes.

• Collaborate with Clinical Program Managers and the clinical team to implement enhanced care coordination when required, which may include peer support and other community-based services designed to stabilize the consumer's condition.

• Assist consumers transitioning from 24-hour care and refer them to support groups as needed.

• Advocate for consumers and families throughout the treatment process and facilitate their involvement in all operational phases of Optum and the Health Plan.
