Job Summary

The Clinical Dietitian is a medical nutrition expert responsible for planning, developing, and directing all nutritional care activities for assigned Veterans. This role supports the facility's mission and coordinates nutritional care with other healthcare providers, the Veteran, the Veteran's caregiver, and the community.

Key Responsibilities
  • Conduct comprehensive nutrition assessments using the Nutrition Care Process (NCP) for assigned patients.
  • Develop and implement individualized plans for Medical Nutrition Therapy (MNT) in accordance with patient medical program goals and objectives.
  • Perform Nutrition-Focused Physical Exams (NFPE) to determine risk and/or degree of malnutrition.
  • Calculate nutrient requirements and necessary diet restrictions or food/drug interactions.
  • Evaluate patterns and relationships among data obtained and possible root causes for nutrition problems.
  • Tailor diet prescriptions to each individual patient according to physical and metabolic needs.
  • Design nutritional treatment plans with consideration for psychosocial, behavioral, physical activity, ethnic, and cultural factors to obtain desirable outcomes and promote long-term adherence.
  • Prioritize the importance of nutrition problems for optimal health, recovery from illness, management of chronic disease, or disease prevention.
  • Utilize evidence-based nutrition practice guidelines and Whole Health Principles to develop shared goals with the patient and formulate a plan of care focused on the etiology of the nutrition problem.
  • Support veteran-centric food service operations with a food-forward approach through knowledge of computer software programs related to food service systems and interface with the food service section to ensure patient satisfaction with meals, assist in implementation of new initiatives, and act as a liaison between clinical units/wards and the food service section.
  • Work cooperatively with foodservice staff to assure conformance to diet prescriptions.
  • Identify patients in need of nutritional support and recommend the appropriate mode of support, including suitable route, formula, and rate of administration.
  • Provide initial and ongoing nutritional assessment of patients receiving enteral and/or parenteral nutrition.
  • Order EN/PN if scope of practice allows.
  • Evaluate and monitor the effectiveness and outcome of MNT interventions.
  • Evaluate the patient's response to care based on identified, measurable outcomes and review the plan as appropriate.
  • Document nutrition care plan in the patient's medical record, including nutrition assessment data, MNT initiated, recommendations, results of nutrition counseling, patient consents, and discharge planning.
  • Measure outcomes through selecting indicators relevant to the nutrition diagnosis.
  • Evaluate the effectiveness and measure progress toward outcomes of nutrition interventions.
  • Documentation supports an interdisciplinary approach and continuity of care.
Nutrition Education and Coordination of Care
  • Assess educational needs and provide nutrition education/counseling for individuals and groups, incorporating a learning approach that considers the cultural, religious, educational, and medical situation of the patient.
  • Include information regarding budgeting, menu planning, food purchasing and preparation, food storage and sanitation.
  • Utilize motivational interviewing techniques to evaluate patient and/or caregiver's readiness to learn and potential for changing behaviors.
  • Evaluate achievement of learning objectives by the patient and/or family.
  • Adapt nutrition education materials and counseling techniques to the patient's level of understanding, which frequently involves simplifying complex dietary restrictions.
  • Identify drug-nutrient interactions and counsel/communicate with the patient regarding the impact on nutrition status and/or nutrient utilization.
  • Utilize virtual care technology such as VA Video Connect and Secure Messaging.
  • Provide follow-up nutrition education/counseling in accordance with the patient's treatment goals and determine when a problem requires consultation or referral to another provider or service.
  • Participate in planning and education of home nutrition support.
  • Function as a clinical preceptor for the dietetic internship program and in a teaching role for other healthcare professionals, patients, and the community.
  • Assist in the education and oversight of dietetic students and interns.
  • Assist in planning learning experiences, teaching, and evaluating performance.
  • Develop and prepare Healthy Teaching Kitchen presentations.
  • Complete MDS/Care plans timely.
  • Participate in community projects and education.
Work Schedule

Monday-Friday, 8am-4:30pm (flexible). Weekends on a rotating basis.

Telework

Available 1-2 times per pay period.
