Advanced Practice Provider in Family Medicine

Position Overview:
The Advanced Provider plays a crucial role in supporting the physicians within the practice by engaging in clinical tasks, surgical procedures, and direct patient care, which encompasses diagnostic, therapeutic, and preventive services. This role is designed for Physician Assistants and Nurse Practitioners who operate under the direct oversight of the practice's physicians.
Key Responsibilities:
- Conduct comprehensive patient histories and physical assessments.
- Identify, treat, and manage both acute and chronic health conditions.
- Request and interpret laboratory and imaging studies.
- Prescribe medications, including controlled substances, in accordance with collaborative practice agreements.
- Accurately document evaluations, management plans, and patient education in medical records.
- Exhibit strong communication skills to effectively relay findings and treatment strategies to patients and their families.
- Evaluate the psychosocial, emotional, and environmental needs of patients and their families.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Provide telephonic triage to assess patient needs and conditions, responding appropriately.
- Coordinate care activities with other healthcare team members, hospitals, or community resources.
- Educate patients, families, and community groups on health-related topics.
- Adhere to documentation and coding standards.
- Continuously update skills and knowledge in the field.
- Work collaboratively with the healthcare team to ensure patient-centered and high-quality care through best practices and case management.
- Participate in scheduled team meetings to clarify roles, responsibilities, processes, and patient care issues.
- Engage in care management by liaising with community agencies as necessary.
- Take part in quality improvement initiatives and committees.
- Conduct hospital rounds to address the needs of the practice.
- Fulfill on-call responsibilities, including emergency and telephone consultations, to support practice requirements.
Required Knowledge, Skills, and Abilities:
- Ability to exercise independent judgment effectively.
- Exceptional service orientation and communication abilities.
- Commitment to maintaining confidentiality at all times.
- Strong written and verbal communication skills.
- Demonstrated clinical proficiency as per organizational standards.
- Current BLS Certification.
- Compliance with continuing medical education (CME) requirements.
- Professional appearance and conduct.
Qualifications:
- Valid New York State license and board certification.
- Completion of an accredited Nurse Practitioner or Physician Assistant program.
- Current BLS Certification.

Work Environment:
