Advanced Practice Provider in Family Medicine

The role of the Advanced Practice Provider is to support the physicians within the practice by engaging in clinical tasks, surgical procedures, and various aspects of direct patient care, which encompass diagnostic, therapeutic, and preventive services. Both Physician Assistants and Nurse Practitioners operate under the direct oversight of the practice's physicians.

Key Responsibilities:

  • Conduct comprehensive patient histories and physical assessments.
  • Identify, treat, and manage both acute and chronic health conditions.
  • Request and interpret laboratory and imaging studies.
  • Prescribe medications, including controlled substances, as permitted by collaborative practice agreements.
  • Accurately document evaluations, management plans, and patient education in medical records.
  • Exhibit effective communication skills with patients and their families to relay findings and treatment strategies.
  • Evaluate the psychosocial, emotional, and environmental needs of patients and their families.
  • Collaborate with other healthcare professionals to ensure continuity of care while addressing physician needs.
  • Provide telephonic triage to assess patient conditions and respond appropriately.
  • Coordinate care activities with other office, hospital, or community members.
  • Educate patients, families, and community groups on health-related topics.
  • Adhere to documentation and coding standards.
  • Continuously update skills and knowledge in the field.
  • Work collaboratively with the healthcare team to deliver patient-centered and high-quality care by adhering to best practice standards and case management principles.
  • Participate in scheduled team meetings to clarify roles, responsibilities, processes, and patient-related issues.
  • Engage in care management by liaising with community agencies as necessary.
  • Contribute to quality improvement initiatives and committees.
  • Conduct hospital rounds to address the practice's needs.
  • Fulfill on-call responsibilities, including emergency and telephone consultations, to support the practice.

Required Knowledge, Skills, and Abilities:

  • Ability to exercise a high degree of independent judgment.
  • Exceptional service orientation and communication abilities.
  • Maintain the highest level of confidentiality.
  • Strong written and verbal communication skills.
  • Demonstrate clinical proficiency as per organizational standards and protocols.
  • Current BLS Certification.
  • Fulfill continuing medical education (CME) requirements.
  • Exhibit a professional appearance and demeanor.

Education and Experience Requirements:

  • Valid NYS license and board certification.
  • Completion of an accredited Nurse Practitioner or Physician Assistant program.
  • Current BLS Certification.
