Job Summary

We are seeking a skilled Licensed Professional Counselor to join our team at FourEight Transitions LLC. As a Licensed Professional Counselor, you will provide trauma-informed care to clients, assessing their mental health and developing individualized treatment plans. You will work collaboratively with internal and external care team members to support clients' well-being.

Key Responsibilities
  • Provide trauma-informed counseling services using evidence-based assessments and interventions.
  • Develop and implement treatment plans with measurable objectives based on diagnostic assessments following DSM-5-TR standards.
  • Maintain CMS-compliant documentation and ensure timely, accurate record-keeping in our Electronic Health Records (EHR) system.
  • Assess clients' mental and behavioral health conditions and collaborate with internal and external providers.
  • Conduct crisis interventions as needed and make appropriate referrals.
  • Uphold HIPAA compliance to maintain a safe and confidential environment for staff and clients.
  • Build positive rapport with a diverse patient population, ensuring a respectful and inclusive approach.
  • Support our program goals by helping reduce cultural and socioeconomic barriers to care.
  • Contribute to a supportive, trauma-informed, and collaborative clinic culture.

Requirements
  • Master's degree in counseling from an accredited university.
  • Unrestricted Oregon License as a Licensed Professional Counselor (LPC).
  • LPCs from other states are encouraged to apply (upon obtaining Oregon licensure).
  • Preferred: 1+ years of experience in an outpatient behavioral/mental health setting.
  • Preferred: Experience working with clients from diverse socioeconomic backgrounds.
  • Preferred: Experience working with complex mental health disorders (e.g., BPD, PTSD, OCD, CPTSD).
  • Strong communication skills and proficiency in Microsoft Office Suite and EHR systems.
