Job Summary

Key Responsibilities
  • Identify Resident Care Requirements: Establish personal rapport with potential and actual residents and other persons in a position to understand care requirements.
  • Monitor and Chart Information: Ensure the health and well-being of residents by monitoring and charting information, including the administration of medications and overseeing staff.
  • Establish a Compassionate Environment: Provide emotional, psychological, and spiritual support to residents, friends, and families.
  • Promote Resident Independence: Establish patient care goals and teach residents, friends, and family to understand their condition, medications, and self-care skills.
  • Assure Quality of Care: Measure health outcomes against patient care goals and standards and make or recommend necessary adjustments.
  • Resolve Resident Problems and Needs: Utilize multidisciplinary team strategies to resolve resident problems and needs.
  • Maintain a Safe and Clean Working Environment: Comply with procedures, rules, and regulations and call for assistance from healthcare support personnel.
  • Protect Facility Residents and Employees: Adhere to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
  • Document Resident Care Services: Chart in resident and department records.
  • Maintain Resident Confidence and Protect Operations: Keep information confidential.
  • Maintain Professional and Technical Knowledge: Attend educational workshops, review professional publications, establish personal networks, and participate in professional societies.
  • Maintain a Cooperative Relationship among Healthcare Teams: Communicate information, respond to requests, build rapport, and participate in team problem-solving methods.
Requirements
  • At least one year of experience in a Long Term Care environment is highly preferred.
  • Must be a graduate of an approved college/school nursing program.
  • Must possess and maintain a current state license as a professional LPN.
