Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will play a key leadership position on our patient-centered medical team, providing advanced care coordination for complex patients and serving as a clinical resource to both patients and internal staff.

Key Responsibilities
  • Lead Care Coordination: Develop and implement individualized care plans for complex patients, ensuring seamless transitions and optimal outcomes.
  • Clinical Resource: Provide expert clinical guidance and support to patients, families, and healthcare teams, promoting evidence-based practice and quality care.
  • Communication Liaison: Serve as the primary point of contact for patients and families, ensuring timely and effective communication and addressing concerns in a compassionate and empathetic manner.
  • Chart Management: Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements and organizational standards.
  • Direct Patient Care: Provide high-quality, patient-centered care, including assessment, planning, implementation, and evaluation of care plans.
  • Quality and Safety: Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of evidence-based practices.
