About the Role

We are seeking a highly skilled Nurse Care Coordinator to join our Emergency team at the University of Washington Medical Center - Montlake. As a key member of our interdisciplinary healthcare team, you will play a critical role in achieving coordinated patient flow through an inpatient stay, culminating in a safe and timely discharge.

Key Responsibilities
  • Assess and screen patients being admitted to identify high medical needs and develop effective care plans.
  • Lead medical team and nursing huddles to ensure timely completion of consults, procedures, and tests, and facilitate coordination between providers across the continuum of care.
  • Develop and monitor outcome measures related to care coordination and staff and patient satisfaction with discharge planning.
  • Collaborate with interdisciplinary teams to remove barriers to discharge due to medical complications and/or system or team obstacles.
Requirements
  • Master's degree in Nursing or related field.
  • Three years of professional nursing experience, with experience in Case Management strongly preferred.
  • Able to communicate effectively in English, both verbally and in writing.
  • Effective interpersonal skills, leadership skills, and clinical expertise.
  • Ability to access information and document in electronic medical records.
  • Current Washington State RN licensure at time of hire, and current BLS.
