About the Role

As a Private Duty Nurse - LVN, you will be responsible for providing and documenting skilled nursing care to our patients in the comfort of their own homes. You will work closely with our patients, their families, and our interdisciplinary team to develop and implement personalized care plans that meet their unique needs and goals.

Responsibilities
  • Implement and document physicians' prescribed plans of care
  • Conduct patient assessments and coordinate care with our interdisciplinary team
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in our electronic system of record
  • Provide health education and training to patients and their families
  • Collaborate with our Case Managers to ensure seamless care transitions
Requirements
  • Active, unencumbered LPN/LVN license in the state of practice
  • Current CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • 6 months prior hands-on nursing experience preferred
