Nurse Practitioner

Responsibilities- Provide high-quality patient care, utilizing the Synergy Model for Care to personalize care for each patient/family.
- Collaborate with the healthcare team to develop and implement effective care plans.
- Conduct thorough assessments, identify patient needs, and develop strategies to address them.
- Communicate effectively with patients, families, and colleagues to ensure seamless care delivery.
- Document patient care accurately and efficiently, utilizing electronic health records.
- Valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.
- Graduate of an accredited school of nursing with 12 months of recent nursing experience within the past five years or recent completion of a re-entry nursing program.
- BLS certification required; ACLS may be required in designated departments.
