Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at CenterWell Senior Primary Care. As a key member of our primary care team, you will provide high-quality, patient-centered care to seniors in their homes, focusing on proactive, preventive care and addressing the physical, emotional, and social well-being of our patients.
Responsibilities- Provide comprehensive primary care services to seniors in their homes, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
- Develop and implement personalized care plans to address the unique needs and goals of each patient.
- Collaborate with our interdisciplinary care teams to ensure seamless coordination of care and optimal patient outcomes.
- Stay up-to-date with the latest clinical guidelines and best practices in primary care, and apply this knowledge to improve patient care and outcomes.
- Participate in quality improvement initiatives to enhance the overall quality of care and patient satisfaction.
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree.
- Active, unrestricted state license to practice as a Nurse Practitioner.
- Board certification as a Nurse Practitioner.
- Minimum 2 years of experience as a Nurse Practitioner in a primary care setting.
- Strong technical skills, including proficiency in electronic medical records (EMRs) and other healthcare technology.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Experience working in a house call or home health setting.
- Knowledge of value-based care and HEDIS quality metrics.
- Experience with urgent care, primary care, family practice, or internal medicine.
