Fallon Health operates the largest PACE program in New England and the fifth largest in the country. Summit ElderCare provides a welcome alternative to nursing home care, offering participants access to medical services and social support at a Summit ElderCare site.

Job Summary

We are seeking a skilled Home Care Nurse to join our team. As a Home Care Nurse, you will be responsible for delivering direct nursing care to PACE participants in any setting, utilizing the nursing process and adhering to standards of nursing practice.

Responsibilities
  • Implement provider orders and conduct nursing assessments according to policy guidelines.
  • Involve participants and their significant others in care planning and delivery.
  • Deliver care to participants in the home setting, including skilled services, based on individualized needs and age-appropriate nursing standards.
  • Monitor and evaluate therapeutic interventions and participate in the development and ongoing review of each participant's care plan.
  • Delegate and accept care responsibilities to other members of the healthcare team, as needed.
  • Identify emergency situations and initiate appropriate nursing orders/interventions.
  • Document all aspects of participant care, including significant changes in health status, monthly nurses' notes, and healthcare teaching in the medical record.
Qualifications
  • Graduate of an accredited school of nursing.
  • Current license as a RN in the state of Massachusetts.
  • CPR and Alzheimer's certification, or willingness to be certified within 60 days of hire.
  • At least two years of recent experience in the direct care of adults or chronically disabled persons, with at least one year caring for a frail or elderly population.
