Key Responsibilities:

  • Deliver high-quality patient care through home visits, following a designated care plan and collaborating with healthcare teams.
  • Administer medication, perform wound care, and provide treatments and procedures as needed.
  • Monitor patient conditions, reporting changes to Clinical or Client Services Manager.
  • Accurately document observations, interventions, and evaluations using a touch pad tablet.

Requirements:

  • Graduation from an accredited nursing program.
  • Current license as a Registered Nurse in Massachusetts.
