HealthEcareers - Client operates a highly-rated home care agency recognized for its quality and excellence. We pride ourselves on being the largest skilled home care provider in the region. Our organizational structure is designed to offer Home Health Nurses various support systems that are often unavailable in other agencies. This includes a field preceptor, IT support, a dedicated scheduler, and a clinical manager, all aimed at ensuring our nurses feel supported and not isolated in their roles.

As a Home Health Registered Nurse, you will be an integral part of our Clinical Team, delivering outstanding, expert care within your community. This role allows for independent practice, focusing on medical interventions in the comfort of patients' homes. You will have the opportunity to build meaningful relationships with patients and their families while enjoying flexibility in your schedule that is typically not available in hospital settings. You will be assigned a specific geographic area, minimizing travel distances compared to other agencies.

This position offers a flexible weekend schedule, with options for 3 12-hour shifts, 3 10-hour shifts, or 3 8-hour shifts. Commitment to working every other weekend is required, with weekdays off during the alternate weeks, or the option to work every weekend with one weekday of flexibility. These schedules are available following the completion of initial training and orientation.

POSITION SUMMARY
This role involves assessing, planning, implementing, evaluating, and documenting nursing care for patients in line with organizational policies and professional nursing standards. You will be accountable for the quality of nursing services provided by yourself and those under your supervision.

CORE FUNCTIONS
1. Assess the physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs of patients. Ensure thorough review of patient history and physical assessments within the required timeframe.

2. Develop a comprehensive care plan, including discharge planning, utilizing assessment data and input from patients, families, and the healthcare team. Goals should be realistic, measurable, and collaboratively established.

3. Implement the care plan through direct patient care and coordination of the healthcare team’s activities. Ensure compliance with policies, procedures, and regulatory requirements while promoting continuity of care.

4. Evaluate the responses of patients and families to care and education, adjusting the care plan as necessary based on ongoing assessments.
