Job Summary

The Roosevelt Park Case Manager is responsible for providing comprehensive support services to help individuals and families participating in the Rapid Rehousing Program. This position requires a proactive and responsive individual who can manage their time well and demonstrate strong organizational abilities. The Case Manager will participate in our collective effort to end and prevent homelessness, working collaboratively with participants, program leadership, and community resources to achieve this goal.

About the Role

The Roosevelt Park Case Manager will maintain a caseload of 20+ participants, providing comprehensive case management services, including initial eligibility verification and enrollment, ongoing needs and status assessments, budgeting, information and referral, crisis de-escalation, safety planning, risk assessment, and mandated reporting. This role requires a strong understanding of evidence-based practices and the ability to apply them consistently. The Case Manager will also participate in property management meetings, collaborate with other team members, and maintain thorough and accurate documentation of participant services.

Requirements

  • 1 year paid or volunteer experience in social work or related human services required
  • Bachelor of Arts in Social Work (BASW) or related human services degree preferred
  • Proficient in Microsoft Office suite
  • Valid CA driver's license and insurance, and reliable transportation (Ability to be added onto company insurance)
