Nurse Practitioner

We are seeking a skilled and compassionate Nurse Practitioner to join our team at SSM Health. As a key member of our healthcare team, you will provide high-quality patient care and support to our residents.
Responsibilities- Provide skilled nursing care to residents according to established care plans and federal and state regulations.
- Administer medications and treatments according to standards of care and maintain competency in all required nursing skills.
- Provide guidance and direction to ancillary staff and monitor the workflow of the unit to ensure coordination of care.
- Use the nursing process for assessment, nursing diagnosis, planning, intervention, and evaluation, and report significant changes to physicians as indicated.
- Ensure accurate inventory of medications at the beginning and end of each shift and distribute medications to residents according to the 5 rights.
- Communicate with residents, guardians, and family members on the current condition and needs of residents in accordance with regulations and standards of practice.
- Provide teaching and training to residents and families as needed.
- Ensure that preventative measures are in place, such as specialty mattresses, foam boots, weekly skin checks, turning/repositioning schedules, toileting schedules, and cushions in all wheelchairs.
- Respond promptly to alarms and ensure that restraint alternatives are in place/activated, including personal alarms, cushion alarms, wheelchair belts, tumble mats, alarm mats, bolstered beds, and hip protectors.
- Provide and ensure quality end-of-life care, using comfort baskets to meet resident and family needs, and ensure hygiene, comfort, and spiritual needs are met.
- Work in a constant state of alertness and safe manner, and may perform point-of-care testing according to policies and procedures.
- Graduate of an accredited school of nursing or education equivalency for licensing.
- No experience required.
