About the Role

We are seeking a skilled and compassionate Registered Nurse to join our team as an Admissions Nurse in our Hospice program. As a key member of our interdisciplinary team, you will play a vital role in coordinating the admissions process for our patients, ensuring they receive the highest quality care in the comfort of their own homes.

Responsibilities
  • Coordinate admissions of hospice patients, conducting skilled assessments of their physical, psychosocial, and spiritual needs.
  • Establish the initial plan of care with the attending physician, medical director, and interdisciplinary team.
  • Provide skilled interventions and instructions to patients and caregivers to optimize symptom control and patient comfort under the direction of the physician.
  • Function as a liaison between disciplines and facilities during the admission process.
  • Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness, and according to policies.
Requirements
  • Current RN License.
  • One year of clinical RN experience required.
  • Current CPR certification.
