Hospice Nursing Opportunity
Catholic Health Initiatives is seeking a skilled Hospice RN to provide compassionate care to patients in their homes, places of residence, or community sites. As a Hospice RN, you will incorporate a broad spectrum of knowledge to provide client care, integrating family, caregiver, environmental, and community resources to promote optimal client and family well-being.
Key Responsibilities:
- Assess and develop a plan of care through interviews with patients and primary caregivers/family, performing physical assessments and integrating them with previous health and psychosocial history.
- Provide case management and document skilled care, treatments, and diagnostic procedures.
- Complete and document initial evaluations of assigned patients, assessing patient status and skilled nursing needs on admission and each subsequent visit.
- Prepare recertification, discharge summaries, and all federally required data entry and/or forms for Medicare.
- Regularly evaluate patient needs and inform physicians and supervisors of changes in patient condition, providing necessary revisions in the plan of treatment.
- Makes referrals as appropriate and authorized by physicians, coordinating services through consultation and interdisciplinary conferences.
Requirements:
- Current RN license issued by the State of Oregon.
- American Heart Association BLS for Health Care Providers within 30 days of hire.
- Valid Oregon Driver's License, automobile insurance, and provide own transportation.
- 1 year acute care nursing experience required, 2 years preferred.
- Recent hospice nursing experience within the last 2 months.
- Associate's Degree in Nursing.
Compensation: $56.74/hour. We are an equal opportunity/affirmative action employer.
